The Delta State Government has constituted an eight-member administrative panel to investigate the circumstances surrounding the death of a Deputy Director in the Ministry of Basic and Secondary Education, Mr Adeda Ekwevugbe.
Ekwevugbe reportedly died at his workplace under circumstances described by the state government as unclear, prompting concerns among his colleagues, organised labour and other sections of the state public service.
The Secretary to the State Government, Dr Kingsley Eze Emu, disclosed this in a statement issued on Thursday, September 25, 2025.
According to him, the police have commenced investigation into the incident, while the government, in consultation with organised labour, decided to constitute the administrative panel to further examine the circumstances surrounding the death.
The panel is chaired by Mrs Charity Ehimen, Permanent Secretary, Office of the Head of Service.
Other members are Mr Mamuzo Erebe, SAN, Solicitor-General and Permanent Secretary, Ministry of Justice; Mr Lucky Nkemachor, Permanent Secretary, Office of the Secretary to the State Government; and Sir A.E. Oghoro, Executive Assistant to the Governor on Public Service.
Representing organised labour are Comrade Goodluck Ofobruku, Chairman, Nigeria Labour Congress, Delta State; Comrade Chief Benson Efeotor, Chairman, Joint Negotiating Council; Comrade Wilson Asekutu, Chairman, Trade Union Congress; and Comrade Dennis Chuks Isichei, Secretary, Joint Negotiating Council, who will serve as Secretary to the panel.
The government gave the panel two weeks to complete its assignment and establish the circumstances surrounding Ekwevugbe’s death.
The findings, the government said, would enable it to address all issues arising from the incident in accordance with the law.
The state government also commiserated with the deceased’s family and assured civil servants of its commitment to ensuring a safe working environment and upholding the dignity and sanctity of life.
